School Vaccination Clinic 2020-2021

MJ’s Natural Pharmacy has partnered with public health to offer school-age immunizations which include:

Kindegartan (age approx. 5): Tdap-IPV (Boostrix Polio or Adacel Plio) + 2nd dose of MMRV (Proquad)
Grade 6 (age approx. 11): HPV9 (2 doses of Gardasil 6 months apart)
Grade 9 (age approx. 14): Men-ACYW-135 (Menveo or Menactra or Nimenrix) + Tdap (Adacel or Boostrix)

This service will be offered at MJ’s Pharmacy #1 and MJ’s Pharmacy #2 (addresses below).

The preferred method to register is to first call us at 604-324-1293 or email: gchong@mjspharmacy.com

Please provide the following information of the child: full name, date of birth, PHN/Carecard#, vaccinations. Please also provide a callback number.

This information will be submitted to public health to request the required vaccinations. Our staff will then arrange an appointment with you and your child for the vaccines, usually about 1 week later.

Please download and fill out the 2 required forms below and bring them to the appointment signed.

All students must show their BC Care Card at the appointment. If your child is not eligible, the appointment may be cancelled. Please check your child’s eligibility and learn more about the immunizations here:

For your child’s safety and the safety of our staff, it is required that all students wear a mask to the appointment. If your child needs one, they are available for free at the pharmacy.

**If your child has any signs and symptoms of illness (fever, sore throat, cough, respiratory issues, etc)，have travelled outside Canada (including the U.S.), or has come into contact with with a COVID-19 positive individual within the past 14 days please refrain from booking and isolate at home for the safety of others. You are welcome to re-book at a later date.
